Phase 3: MLX outlier rejection and LLt/MIR

Part of epic #278. Port do_reject (good_idx mechanism) and the LLt/MIR diagnostic outputs to AMICAMLXNG, honoring the #157 decision (Fortran-faithful stale LLt from stashed logV) once that lands in torch/numpy.
