[post Oct] `Diagnosis Type` in clinical download should be `Condition Interpretation`

In the clinical data download, Diagnosis Type should be Condition Interpretation - see all_cohorts_condition.csv.

The Condition Interpretation for all DSC conditions should be "Self-reported", not "Clinical".

We might need to review the whole clinical data download fields/format at some point.
