INSERT into `types_members` ignores `offset`

At https://github.com/allthingsida/idasql/blob/main/src/lib/src/types_members.cpp#L364 argv[4] (offset) is never used, so a member is always INSERTed at the end no matter which offset is provided

Fixed for v0.0.18 (coming shortly). INSERT honors the explicit offset (and offset_bits) — the member lands at the requested offset, guarded by a bit-accurate overlap check, so an overlapping insert is rejected rather than silently appended. Thanks @shrddr!
