camera_cards -f mkv for AVCHD error

Encountered the following error when running camera_cards -f mkv for AVCHD cards. Full terminal output attached.

Found no issue with the resulting concatenated mkv file...

this is related to issue #69 and #79

For this, I suggest that when the remux is done that we make a sidecar qc report that lists where the packet errors are.

tested camera_cards -f mkv and it is not generating the log file as when running with -N (issue #85 )
